The Wilkes County Youth Survey will be available to students in grades 7–13. Parent/guardian consent is required before a student can participate.
Parents/Guardians: Consent forms are available through your Infinite Campus Parent Portal.
To complete the form:

- You should see Documents
- Look for the Youth Survey consent form
- Review the information
- Submit your response
